Surgery Deposit Agreement

This agreement is made between Dr. Ufuk Askeroğlu Clinic and the patient to define the terms and conditions regarding the surgery deposit payment.

1. Deposit Payment and Refund Policy

1.1. The patient agrees to pay a deposit, which is a portion of the total surgery cost, to confirm their surgery appointment.

1.2. The deposit payment serves as a confirmed reservation for the surgery and covers the surgical preparation process, photo analysis, and human/hour costs of clinical staff, doctors, etc. It is considered a consultancy fee and is therefore strictly non-refundable under any circumstances.

2. Cancellation Policy

2.1. If the patient requests a cancellation for any reason, the deposit will not be refunded. The deposit will be retained by the clinic to cover service and planning costs without any deductions.

2.2. If the patient cancels 24 hours before the surgery or does not attend the surgery appointment without a valid reason, they will be responsible for paying the full surgery fee.

3. Surgery Date Change

3.1. If the patient wishes to reschedule the surgery, they must submit a written request to the clinic at least 21 days in advance.

3.2. The rescheduling of the surgery is subject to the clinic’s availability. If no suitable date is found, the deposit will remain non-refundable, and the patient may need to make a new deposit for a future surgery date.

4. Force Majeure

4.1. If the patient must cancel the surgery due to force majeure (such as a terminal illness, severe accident, etc.), they must provide valid documentation. The clinic will assess the case, but the deposit remains non-refundable.

4.2. Aside from force majeure cases, all cancellations and changes made by the patient are subject to the terms of this agreement, and the deposit remains non-refundable.

5. Final Provisions

5.1. By signing this agreement, the patient acknowledges that they have read, understood, and accepted all the terms outlined above, including the strict non-refundable policy.

5.2. Any disputes arising from this agreement shall be resolved by the Istanbul Courts and Enforcement Offices.
