Online Consultation Agreement

This agreement is made between Dr. Ufuk Askeroğlu Clinic and the patient to define the terms and conditions regarding the online consultation services provided by the clinic.

1. Consultation Payment and No Refund Policy

1.1. The patient agrees to pay the full consultation fee prior to the scheduled online consultation.

1.2. The consultation fee is strictly non-refundable under any circumstances. This fee covers the consultation preparation, analysis, and the time of the clinical staff, making it a consultancy fee.

2. No Cancellation or Refunds

2.1. Once the consultation fee is paid, the patient acknowledges that they will not be entitled to any refunds, regardless of whether they cancel, fail to attend, or choose not to proceed with the consultation.

2.2. The clinic does not provide refunds due to schedule conflicts, personal reasons, or any other circumstances outside of force majeure cases as outlined below.

3. Photos and Documents

3.1. The patient agrees that the clinic may take photographs, videos, and other media of them during the consultation for medical analysis and documentation purposes.

3.2. The patient acknowledges and agrees that any photos, videos, medical documents, or other materials provided by them to the clinic (whether before, during, or after the consultation) will be stored and used by the clinic for medical, consultation, and administrative purposes only.

3.3. All photos, videos, and documents shared with the clinic by the patient will remain confidential, and the clinic will not share them with any third party without prior consent unless required by law or for medical purposes.

4. Force Majeure

4.1. If the patient must cancel due to force majeure (such as a terminal illness, severe accident, etc.), they must provide valid documentation for special evaluation. Even in such cases, the decision regarding a refund will be made solely at the clinic’s discretion, and the clinic is not obligated to issue any refunds.

5. Final Provisions

5.1. By signing this agreement, the patient acknowledges, understands, and fully accepts that the consultation fee is 100% non-refundable under any circumstances.

5.2. The patient confirms that they have read and agreed to the terms of this agreement before making the payment.

5.3. Any disputes arising from this agreement shall be resolved by the Istanbul Courts and Enforcement Offices.
